Evaluation of newborn adaptation immediately after birth
⚠️ Warnings and Limitations
Contraindications:
- •Do not wait for APGAR to start resuscitation if needed
- •Score influenced by maternal medications, prematurity, congenital anomalies
Limitations:
- •Does not replace complete ABC assessment
- •Subjective for some parameters (color in neonates of different ethnicities)
- •1 minute score: condition at birth
- •5 minute score: response to resuscitation

📚 Special Notes
- 🔴 CRITICAL: APGAR should NOT DELAY or GUIDE resuscitation initiation. If newborn not breathing → START CPR IMMEDIATELY, assign score AFTER. Score is for DOCUMENTING response, NOT deciding whether to start (ACOG 2015)
- ⏱️ Assess at 1 minute (baseline) and 5 minutes (response to intervention)
- ⏱️ If <7 at 5 min: continue every 5 min until 20 min
- 📊 APGAR does not predict long-term neurological outcome
- 📝 Always document time and score for medicolegal purposes
